What causes the horrible scent from the mouth and or noise even though care and attention has been given?

Hmmm. In my experience, it's usually dental problems. Rotting teeth or gums can be very smelly. Some diseases can also lead to bad breath: diabetes has a unique and sometimes unpleasant smell, and pancreatitis (or cancer) can overstimulate enzyme production and result in fetid breath.

I'd definitely start with the teeth though :)
