This is a very complex question. Treatment for diabetes needs to be individualized for every person. In general people with type 1 diabetes will need to be on insulin. People with type 2 diabetes often do not need insulin, but sometimes they do.

Because having high blood sugars damages the body in so many ways, if you have diabetes it is crucial that you get it under control. You need to make sure you get to and maintain a healthy weight, exercise on a regular basis, and eat healthy. It is also very important that people with diabetes get other medical conditions like thyroid problems, high blood pressure and high cholesterol under control.

Diabetes type 2 can be controlled by diet and exercise, taking oral meds and yes Dr Jeff, we do take insulin!
